Injection therapy can be a very effective treatment as part of a physiotherapy rehabilitation for conditions such as frozen shoulder, tennis elbow or joint osteoarthritis or it can also be used as a stand alone treatment for conditions such as trigger finger or De Quervains Tenosynovitis (wrist tendon pain).

Who would be suitable for injection therapy?

You may be a candidate for injection therapy if you’re still having pain and more conservative options have failed, or if you need some pain relief and mobility in order to effectively do rehab and physiotherapy.

Are there any side effects?

Depending on which type of injection/medication used there might be some side effects. This will be thoroughly discussed with your physiotherapist prior to injection

Corticosteroid Injection

Steroid injections (also known as corticosteroid injections) are types of anti-inflammatory medicines, are often recommended for people with painful conditions such as acute flare up of osteoarthritis, rheumatoid arthritis, bursitis, frozen shoulder, tendinitis and repetitive strain injuries. The injections are used to reduce inflammation and can also reduce joint swelling and pain which will aid mobility and function of the joint and surrounding soft tissues for a period of time.

What will be injected?

Steroid Injections are often referred to as Corticosteroid Injections and are available in different forms. The affected soft tissue region or joint is injected with the lowest effective doses low a dose as recommended in latest evidence, which should help reduce inflammation locally. Sometimes a local anaesthetic will also be injected with the steroid that acts as a painkiller and may provide immediate temporary pain relief.

How quickly does it take to feel a reduction in symptoms following steroid injections?

When a local anaesthetic is injected, this can reduce your pain within a few minutes of the injection. The steroid affect will take slightly longer to occur, usually 1-2 days but can be up to a week. Within this time, you can continue to manage your pain through gentle movement, analgesia, ice, or heat packs. It is advised that you take a few days of relative rest from vigorous or excessive exercise / stress of the joint / area affected after the injection.

How long do the affects last?

The effects of the steroids should only really last for up to 4-6 weeks, but the benefits can last longer periods of time, particularly if used in conjunction with a rehabilitation program. Steroid injections can promote an anti-inflammatory response which will help to reduce pain and swelling allowing you to focus on your rehabilitation exercises.

Are there any side effects?

This will depend on where the steroid is injected and what type of steroid you receive. Milder forms such as hydrocortisone tend to have weaker side effects than the stronger mixtures of methylprednisolone and triamcinolone.

   – Pain and discomfort for a few days in the injected area

   – Temporary bruising or collection of blood under the skin
   – A flushed complexion
   – An infection, causing redness swelling and pain (very rare)
   – Paler skin at the injected area
   – Loss of fat at the injected area
   – A rise in blood sugar (more likely for diabetes sufferers)
   – Risk of tendon rupture (very rare)

Who cannot have steroid injections?

There are certain reasons why injection would not be a suitable treatment option. Your physiotherapist will give you a call prior to your appointment to ensure there are no contraindication for treatment.

Hyaluronic Acid – Ostenil®

Hyaluronic acid injections are normally used for the treatment of osteoarthritis by reducing pain and improving the function. This type of injection comes without any form of side effects and you are able to return to normal activities immediately after the injection.

It can be injected into the knee, or any of the other synovial joints including shoulders, elbows, wrists, thumbs, knees, ankles, and big toes.

At Bode Clinic we use the brand Ostenil® as our choice of hyaluronic acid, and we are one of their recognised clinics which is carefully selected by the produces to ensure the best quality care for the patients.

From one course you can expect relief for 9-12 months. Ostenil (hyaluronic acid) is widely used in sports medicine, and is used by most UK premiership football clubs, rugby clubs, cricket clubs and the British Olympic Association.

What is Ostenil®?

Ostenil® is a brand name for Hyaluronan Injections, also referred to as Hyaluronic Acid Injections (HA Injections) that contain Sodium Hyaluronate. These injections are used to manage the symptoms of osteoarthritis and help preserve the health of the joint.

How does it work?

Ostenil® is a solution containing Sodium Hyaluronate which is a form of Hyaluronic Acid. It is injected into the space in the joint that contains synovial fluid and works by restoring the normal balance between the breakdown and production of Hyaluronic Acid. This effect of Ostenil® means that it can decrease pain and stiffness and improve the other symptoms of osteoarthritis.

What is Ostenil Plus?

Ostenil Plus contains a higher concentration of sodium hyaluronate (hyaluronic acid) than standard Ostenil, plus mannitol, and offers the option to reduce the number of injections to one per treatment cycle.

How does Ostenil Plus work?

This medication is injected into the space between the joint that contains synovial fluid and works to restore the balance between the breakdown and production of hyaluronic acid. This effect means Ostenil can help decrease pain and stiffness of the joint.

The hyaluronic acid in Ostenil Plus is very pure and is manufactured using fermentation. Containing no animal proteins, it is unlikely to cause an allergic reaction, plus the addition of mannitol (a sugar derivative) helps the hyaluronic acid to work for longer and more efficiently in the joint.

Does Ostenil Plus have any side effects?

Ostenil Plus has been extensively tested and has not been found to cause any serious side effects, plus it contains no animal proteins, so is unlikely to cause an allergic reaction.

As with all injections there is a very small risk of infection and occasional post injection pain at the site, which may last a day or so.

How often can you have hyaluronic acid injected?

The injections can be used on a regular basis if required as they have no known side effects, and they are not known to be detrimental to the body’s tissues in any way. For optimal affect, chronically swollen joints can be injected first with steroid to reduce inflammation and then injected with hyaluronic acid around 4 weeks later, reducing the need for repeated steroid injections.

Botulinium Toxins

Bode Clinic are proud to be one of the very few clinics in Manchester offering medical injections of botulinium toxin to help a range of physical problems paired with physiotherapy rehab/treatment. The combination has shown to be an extremely effective way of tackling chronic conditions like TMJ and other conditions where muscle spasms and tension is severe.

Botulinum toxin type A is an injectable neurotoxin primarily known for the ability to reduce appearance of facial wrinkles, but they are also used to treat conditions such as muscle spasms (caused by conditions like Parkinsons, MS, stroke), it can be used to treat excessive sweating (hyperhidrosis), TMJ and migraines as well as Bell’s Palsy.

The neurotoxin works by blocking certain nerve signals that makes muscles contract, making it a very useful treatment options in many cases.

Are there any side effects?

As this medication is given at the site of your condition, most of the side effects occus close to where the medication is injected. Redness, bruising, infection, and pain at the injection site may occur. Your physio will go through all possible side effects before proceeding with any treatment

How long does the effect of the injection last?

This can vary, anything from 3 months to 2 years. Depending on the treatment given we will always pair this with further rehab if it is appropriate. For hyperhidrosis treatment there is no rehab/treatment after.

What is hyperhidrosis?

Hyperhidrosis is a condition which is causing excessive sweating that’s not always related to heat or exercise, this is caused by overactive sweat glands. Although it is not clear why it develops, it’s thought to be a problem with part of the nervous system (sympathetic) and it is telling the body to produce heavy sweating for no reason. Using injection treatment can significantly reduce sweating and is known to be the most effective treatment when nothing else is helping.

Can botulinum injection help with TMJ?
